Behavioral Breakthrough Assessment Agreement

Step 2 — Assessment Agreement

Please read all terms carefully before signing.

1. Assessment Overview

DePaul K9 Academy will conduct a 1.5-hour in-depth Behavioral Breakthrough Assessment at the client's home. During this session, Carol DePaul will evaluate the dog's behavioral patterns, identify root causes of problematic behaviors, and develop a customized behavior modification plan. The assessment fee of $285 covers this evaluation and plan development only and does not include ongoing training sessions.

2. Behavioral Disclosure & Safety

The client certifies that all behavioral history disclosed in this agreement is accurate and complete. The client agrees to ensure the dog is safely secured and under control at the time of the trainer's arrival. If the dog's behavior during the assessment poses an immediate safety risk to Carol DePaul or others, Carol reserves the right to end the session immediately. In such cases, a partial fee may be charged for time spent.

3. No Guarantee of Results

DePaul K9 Academy does not guarantee specific behavioral outcomes. The assessment provides a professional evaluation and customized plan. Long-term behavioral improvement depends entirely on the owner's consistent application of the recommended training methods and structure. DePaul K9 Academy is not responsible for behavioral regression resulting from inconsistent follow-through after the assessment.

4. Payment Terms

  • Assessment fee: $285
  • Payment is due at the time of the assessment.
  • Accepted payment methods: Cash, Zelle, Venmo, Cash App, or Credit Card (3.5% processing fee applies to credit cards).
  • Military discount (10%) available for active duty, veterans, and first responders with valid ID.

5. Cancellation & No-Show Policy

  • A minimum of 24 hours advance notice is required to cancel or reschedule your assessment.
  • Late cancellations (less than 24 hours notice) and no-shows will forfeit the assessment fee.
  • DePaul K9 Academy reserves the right to cancel or reschedule due to illness, weather, or other unforeseen circumstances.

6. Liability Waiver

The client acknowledges that working with dogs exhibiting behavioral challenges involves inherent risks. The client agrees to hold Carol DePaul and DePaul K9 Academy harmless from any injury, damage, or loss arising from the dog's behavior during or after the assessment. The client assumes full responsibility for their dog's actions at all times and agrees to take all reasonable precautions to prevent injury to the trainer, other people, or other animals.

7. Video Coaching & Public Media Release

Educational Recording & Homework

The client agrees to participate in our video coaching program. This includes recording portions of live sessions for client review and requiring the client to submit 1–2 minute progress videos between lessons.

Public Media and Demonstration

DePaul K9 Academy creates educational content, before-and-after case studies, and training demonstrations for our website and official YouTube channel. By signing this contract, the client grants DePaul K9 Academy full permission to use footage of their dog's training sessions on these public platforms.

Privacy Guarantee

Private session footage and homework clips are used strictly for your instruction and public education. We will never sell your videos or private information to third parties.

By signing this agreement below, the client acknowledges and consents to the Video Coaching & Public Media Release terms stated above.

8. Governing Law & Entire Agreement

This agreement shall be governed by the laws of the State of Florida. This document constitutes the entire agreement between the parties and supersedes all prior verbal or written communications.

Digital Signature

By typing your full legal name below and checking the confirmation box, you acknowledge that you have read, understood, and agree to all terms of this Behavioral Breakthrough Assessment Agreement. Your typed name constitutes a legally binding electronic signature under the Florida Electronic Signature Act and the federal E-SIGN Act.

By submitting, you confirm all information is accurate and your typed signature is legally binding. Carol will receive a copy and will confirm your assessment date and time.
