Board & Train Enrollment

Important — Please Read Before Enrolling

DePaul K9 Academy does not accept dogs with a history of biting humans or children into the Board & Train program. If your dog has a bite history, please do not complete this enrollment. Contact Carol directly at 321-960-0326 to discuss whether a different program may be appropriate.

A 50% deposit is required to secure your program dates. The remaining balance is due on or before program completion.

Agreement Terms

1. Program Overview & Owner Expectations

The client agrees to enroll their dog in the Board and Train program provided by DePaul K9 Academy. The dog will reside in a professional, home-based environment and receive daily structured training, exercise, and care tailored to their behavior modification plan.

  • Foundation Training: The trainer's role is to establish a solid foundation of obedience and behavior modification while the dog is in residential care.
  • Owner Follow-Through: The owner understands that behavior modification is an ongoing process. Long-term success is strictly dependent on the owner's willingness to follow through and remain entirely consistent with the provided structure, safety rules, and training plan once the dog returns home.
  • No Guarantee: DePaul K9 Academy does not guarantee specific behavioral outcomes. The results of this program are only as good as the owner's commitment to consistently applying the training methods and structure as instructed by the trainer after the dog returns home. Continued success requires daily reinforcement by the owner.

2. Transfer of Training Records

Upon successful completion of the Board and Train program, DePaul K9 Academy will provide the owner with a complete training summary including all commands taught, behavioral progress notes, recommended daily structure, and any ongoing training guidance. This documentation is the property of the client and is provided to support continued success at home.

3. Property Damage and Liability Disclaimer

The owner explicitly agrees to assume full financial and legal responsibility for all damage or destruction caused by the dog during the boarding and training period. This includes, but is not limited to, damage to the trainer's home, real property, personal property, fencing, crates, landscaping, or vehicles. The owner agrees to promptly reimburse DePaul K9 Academy for the full cost of professional repair or replacement for any property damage caused by the dog while in the trainer's care.

4. Fees, Deposits, and Payment Terms

  • A 50% deposit is required to secure the program dates.
  • The remaining balance is due on or before the completion of the program.
  • Accepted payment methods: Cash, Zelle, Venmo, or Cash App. Credit cards are accepted with a 3.5% processing fee.

5. Health and Safety

The owner certifies that the dog is current on all required vaccinations (Rabies, DHPP, and Bordetella) and is on active flea/tick prevention. The owner will provide a copy of the vaccination record at the time of drop-off or before the scheduled date. The owner authorizes the trainer to seek emergency veterinary care at the owner's expense should a medical emergency arise during the program.

6. Early Termination

If the program must be terminated early due to illness, injury, aggression, or any other reason, fees will be prorated at the daily rate of $257 per day for days completed. The deposit is non-refundable. DePaul K9 Academy reserves the right to terminate the program at any time if the dog poses a safety risk to people or other animals.

7. Video Coaching & Public Media Release

Educational Recording & Homework

The client agrees to participate in our video coaching program. This includes recording portions of live sessions for client review and requiring the client to submit 1–2 minute progress videos between lessons.

Public Media and Demonstration

DePaul K9 Academy creates educational content, before-and-after case studies, and training demonstrations for our website and official YouTube channel. By signing this contract, the client grants DePaul K9 Academy full permission to use footage of their dog's training sessions on these public platforms.

Privacy Guarantee

Private session footage and homework clips are used strictly for your instruction and public education. We will never sell your videos or private information to third parties.

By signing this agreement below, the client acknowledges and consents to the Video Coaching & Public Media Release terms stated above.

8. Governing Law & Entire Agreement

This agreement shall be governed by the laws of the State of Florida. This document constitutes the entire agreement between the parties and supersedes all prior verbal or written communications. Any modifications must be made in writing and signed by both parties.

Board & Train Agreement — Digital Signature

By typing your full legal name below and checking the confirmation box, you acknowledge that you have read, understood, and agree to all terms of this Board & Train Agreement & Liability Waiver. Your typed name constitutes a legally binding electronic signature under the Florida Electronic Signature Act and the federal E-SIGN Act.

Step 3 — Veterinary Medical Release Form

This release authorizes your veterinarian to provide care and share medical information with DePaul K9 Academy during the Board & Train program.

I, the undersigned, hereby certify that I am the legal owner of the dog named in this enrollment. I give my full authorization to my primary veterinarian, or any emergency veterinary clinic, to provide medical treatment and care to my dog if deemed necessary while under the supervision and care of Carol DePaul and DePaul K9 Academy during their Board and Train program.

  • Financial Responsibility: I assume full financial responsibility for all veterinary fees, diagnostics, treatments, procedures, and medications administered to my dog during this period. I agree to settle all veterinary costs directly with the medical clinic.
  • Communication & Records: I authorize my veterinarian to discuss my dog's medical history, current health status, and treatment options directly with DePaul K9 Academy. I explicitly request that the veterinary clinic send all relevant correspondence, medical updates, or treatment records by email to [email protected].
